OBJECTIVE: To evaluate the olfactory fossae depth according to the Keros' classification and determine the incidence and degree of asymmetry in the height and contour of the ethmoid roof. MATERIALS AND METHODS: Retrospective analysis of 200 coronal computed tomography studies of paranasal sinuses performed in the period between August and December, 2006. RESULTS: According to the Keros' classification, olfactory fossae type II was most frequently found in 73.3% of cases followed by type I in 26.3% and type III in 0.5% of cases. Asymmetry in the ethmoid roof height was found in 12% of cases (24 computed tomography studies), and contour asymmetry was found in 48.5% (97 computed tomography studies), with flattening of the ethmoid roof on one of the sides. CONCLUSION: As regards the olfactory fossae depth, the Keros' type II was most frequently found. In most of cases, the ethmoid roof asymmetry was related to angulation of the lateral lamella of the cribriform plate.

OBJECTIVE: The present study was aimed at evaluating the frequency and types of anatomical variants of the ostiomeatal complex. MATERIALS AND METHODS: Computed tomography studies of 200 patients evaluated for clinical suspicion of sinusopathy were reviewed. RESULTS: Two or more types of anatomical variants were observed in 83.5% of the patients. The most frequent anatomical variants involved the middle turbinate (84%) and the nasal septum (34%). Other common variants found were the presence of pneumatized agger nasi cells and infraorbital ethmoid cells (Haller cells). CONCLUSION: The prevalence of anatomical variants in the ostiomeatal complex is high, the most frequent ones involving the middle turbinate and the nasal septum.

OBJECTIVE: To establish reference values for fetal nasal bone length measurements between 11 and 15 gestational weeks in a Brazilian population. MATERIALS AND METHODS: A cross-sectional study was developed with 171 normal pregnant women between their 11th and 15th gestational weeks. The fetal nasal bone was measured by means of transabdominal ultrasound in all of the cases. The 5th and 95th percentiles for the nasal bone length were calculated by the formula: mean ± 1.645 standard deviation. The Spearman correlation coefficient with 95% confidence interval was utilized to correlate the nasal bone length with fetal anthropometric parameters. RESULTS: The nasal bone could be measured in all of the cases with the mean length ranging between 1.69 mm and 2.94 mm. Nasal bone length demonstrated to be strongly correlated with all of the fetal anthropometric parameters (p < 0.001) and with the gestational age (R² = 0.59). CONCLUSION: Despite the preliminary character of the present study, a reference range of fetal nasal bone length was established.

OBJECTIVE: To correlate the isolated sonographic finding of umbilical cord cyst with fetal anomalies such as chromosomopathies and structural changes. According to the medical literature, the clinical implications of the sonographic finding of umbilical cord cyst in the second and third trimesters of pregnancy are well established; however, the meaning of this finding in the first trimester still remains controversial. MATERIALS AND METHODS: A retrospective study was developed with consecutive, pregnant women with single living fetuses presenting with umbilical cord cyst as an isolated finding, over a 10-year period (1996-2006). Ultrasound studies were performed in all cases for screening of fetal anomalies after the diagnosis of umbilical cord cyst. Neonates and umbilical cords were evaluated after delivery for the presence of abnormalities. RESULTS: Nine cases presenting umbilical cord cyst as a sole finding with no other sonographic marker for fetal abnormality were evaluated. Two cases were detected in the first pregnancy trimester and seven cases in the second and third trimesters. Fetal cytogenetic study was done by means of amniocentesis in two cases. No newborn presented with structural anomalies or aneuploidy. CONCLUSION: Isolated sonographic finding of umbilical cord cyst did not imply increased risk for fetal structural anomalies or aneuploidies.

OBJECTIVE: The present study was aimed at correlating ultrasonographic and magnetic resonance imaging findings in patients with fetal sacrococcygeal teratoma. MATERIALS AND METHODS: Three pregnant women between the 30th and 35th weeks of gestation were submitted to fetal magnetic resonance imaging because of previous ultrasonographic findings suggestive of fetal sacrococcygeal teratoma. Subsequently, they were submitted to ultrasonography for correlation of the imaging findings. Tumors size, location, extent and content were evaluated both at magnetic resonance imaging and ultrasonography. RESULTS: Findings regarding tumor location, size and content were similar for both methods. All the lesions were found in the sacrococcygeal region, with a mean size of 6.0 cm x 9.0 cm. As regards the tumors content, two of them were mixed solid-cystic, and one, entirely cystic. Magnetic resonance imaging was superior to ultrasonography in the evaluation of the exact tumor extent, accurately demonstrating pelvic involvement in all of the three cases. CONCLUSION: Fetal magnetic resonance imaging has showed to be a valuable adjunct to obstetric sonography in the evaluation of fetal sacrococcygeal teratoma, because of its higher accuracy in the determination of these tumors extent and content, playing a significant role in the therapeutic planning and increasing the chances of cure for these fetuses.

OBJECTIVE: To demonstrate the most frequent features of cystic breast lesions at ultrasound elastography, discussing the applicability of this method. MATERIALS AND METHODS: The present casuistic included 150 patients referred for percutaneous breast biopsy of 175 lesions. Histologically diagnosed solid lesions (153 lesions) were excluded; lesions histologically diagnosed as cystic (22 lesions), including complicated cysts, papillary lesions, inflammatory lesions, typical columnar cell hyperplasia and duct ectasia were retrospectively classified by means of elastography, according to a scoring system developed by the authors, with categories ranging between 1 and 4. RESULTS: Thirteen (59%) of the 22 lesions evaluated corresponded to cysts, one (4.6%) to duct ectasia, two (9.2%) to inflammatory lesions, five (22.6%) to papillary lesions, and one (4.6%) to columnar cell hyperplasia. The scoring system was applied with the following results: 17 category 2 lesions, four category 3 lesions, one category 4 lesion, and none category 1 lesion, with a 95% specificity. CONCLUSION: Different features of cystic breast lesions are demonstrated by elastography according to histological results, representing a useful and easily applicable method for differentiating benign from malignant breast lesions.

OBJECTIVE: To evaluate the efficacy of infra-inguinal, primary angioplasty as a method of limb salvage in patients with trophic lesions secondary to critical ischemia. MATERIALS AND METHODS: Thirty-six patients submitted to primary percutaneous transluminal angioplasty without stenting were evaluated. All of them presented critical limb ischemia with trophic lesion (Rutherford grade III, category 5). Eighty-four angioplasties were performed for 17 lesions in superficial femoral artery, 16 lesions in popliteal artery, and 51 lesions in below-knee arteries. Additionally, prevalence in relation to sex, age, limb involved and main comorbidities have been discussed, with technical considerations regarding procedures and materials utilized. RESULTS: Clinical success has been defined as complete healing of the trophic lesion or amputation level limited to toes or forefoot, not requiring bypass surgery, independently from time, drugs utilized and number of debridement procedures performed. CONCLUSION: Femoropopliteal and belowknee angioplasties present high technical success, low mortality and low complications rates and are highly effective in the treatment of patients with critical lower limb ischemia.

OBJECTIVE: Contrast medium-induced nephropathy is the third most frequent cause of iatrogenic acute renal failure involving inpatients. The present study was aimed at evaluating the protective effect of n-acetylcysteine and allopurinol in both male and female rats receiving diatrizoate. MATERIALS AND METHODS: Thirty-five young adult Wistar rats submitted to hydric restriction were divided into groups as follows: groups 1 and 2 (respectively male and female rats) receiving saline solution; groups 3 and 4 (respectively male and female rats) receiving diatrizoate; group 5 (male rats) receiving diatrizoate and n-acetylcysteine; group 6 (male rats) receiving diatrizoate and allopurinol; and group 7 (male rats) receiving diatrizoate and n-acetylcysteine + allopurinol. The glomerular filtration was evaluated by measurement of creatinine clearance. Student's t-test and the test of signal were utilized for statistical analysis. RESULTS: Animal models receiving allopurinol did not present a significant increase in the creatinine levels, while n-acetylcysteine did not prevent the creatinine levels increase. CONCLUSION: Allopurinol has shown to be more effective than n-acetylcysteine in the renal function protection against sodium diatrizoate-induced damages. No difference has been found between male and female groups as regards the intensity of sodium diatrizoate-induced renal damages.

OBJECTIVE: The present study was aimed at comparing indices obtained from dental panoramic radiographs with bone densitometry results in the identification of individuals affected by osteoporosis/osteopenia. MATERIALS AND METHODS: Panoramic mandibular, mental, antegonial, gonial and mandibular cortical indices calculated with basis on dental panoramic radiographs of postmenopausal women were compared with bone densitometry results, twice by one observer and once by a second observer. RESULTS: Significant differences were not found between the two measurements performed by the same observer, except for the antegonial index on the left side. Interobserver agreement for both sides was good. The variance analysis demonstrated statistically significant differences among groups (normal, osteopenia and osteoporosis) for all the indices evaluated. Differences were significant among all mean panoramic mandibular and mental indices. On the other hand, for the antegonial and gonial indices, only the normal group presented higher means as compared with the other groups (osteopenia and osteoporosis) which did not differ between themselves. CONCLUSION: The indices evaluated were reproducible; panoramic mandibular and mental indices presented the highest sensitivity in the detection of osteopenia/osteoporosis, however the panoramic mandibular index specificity was low. Although all the indices evaluated could identify low bone density, only the panoramic mandibular and mental indices could differentiate patients affected by osteopenia/osteoporosis.

Intrathoracic manifestations of collagen vascular diseases are very common. The frequency of intrathoracic manifestations and the pattern of abnormality are variable depending on the type of collagen vascular disease and may simultaneously involve one or more of the following: lung parenchyma, airways, pulmonary vessels, pericardium, and pleura. Most significant manifestations include diffuse interstitial pneumonia and pulmonary hypertension which together represent the main causes of morbidity and mortality of these patients. Pulmonary, airway and pleural involvement may also be secondary to the therapy adopted for management of the disease, or result from bacterial pneumonia or opportunistic infection. In the present review, the authors summarize the main intrathoracic manifestations of collagen vascular diseases and the differential diagnosis on high-resolution chest computed tomography.

The structures responsible for the growth of bones include the physis (also called growth plate) and the epiphysis. Affections involving patients with immature skeletons, i.e., with a still open growth plate, may affect the bone growth, resulting in complications such as growth arrest, limb shortening and angular deformities. Traumatic conditions, many times resulting in epiphyseal fractures, are the most frequent cause of growth plate injuries. A careful evaluation of these patients by means of currently available imaging methods, especially radiography, computed tomography and magnetic resonance imaging, allows an early diagnosis of the involvement of structures related to the bone growth, besides an appropriate management, reducing the probability of secondary complications.

Schwannomas of the sinonasal tract are rare entities that are amenable to local resection. We present a 69 year old woman with a schwannoma of the inferior turbinate that was successfully resected. Schwannomas arising from the inferior turbinate are very infrequent.

Giant cell tumor is the sixth most frequent primary bone neoplasm, affecting long bone metaphysis, most frequently in young adults. On radiological images, this tumor appears as a lytic, well-defined, eccentric lesion. The authors report a case of benign giant cell tumor in a patient who presented with lung metastases five years after undergoing resection of the primary tumor.
