Objective: To evaluate the role of ultrasonography in the assessment of histopathological changes in patients with chronic hepatitis C, with emphasis on hepatic steatosis. Materials and Methods: Liver ultrasonography results were compared with histopathological findings of liver biopsy of 192 patients with chronic hepatitis C virus infection. All the US examinations followed a single protocol, analyzing the following aspects: echogenicity, echotexture and attenuation. The patients sample was divided into two groups as follows: patients with sonographic changes and patients with no sonographic changes. Sonographic findings of both groups were compared with histopathological findings after liver biopsy. Results: Statistically significant intergroup differences were observed just regarding architectural changes grades 0 and 3 and hepatic steatosis. Attenuation was the sonographic criterion that was best correlated with hepatic steatosis. Conclusion: The results of the present study demonstrate that, in patients with chronic hepatitis C, ultrasonography has limitations in the characterization of histopathological changes, with an intermediate rate of agreement with the diagnosis of hepatic steatosis. Considering the specificity of 77.9% and the negative predictive value of 95.5%, the authors highlight the capacity of the method to demonstrate the probability of absence of hepatic steatosis.

Objective: To prospectively compare full dose gadopentetate dimeglumine (Gd-DTPA) with full dose gadobenate dimeglumine (Gd-BOPTA) in the detection of focal hepatic lesions in patients with chronic liver disease on MRI. Materials and Methods: Eight patients with hepatic cirrhosis and a strong suspicion for small hepatocellular carcinoma based on prior MRI underwent contrast-enhanced MR examinations, one with full dose Gd-DTPA and one with full dose Gd-BOPTA. The exams were performed from 72–108 hours apart. Two blinded and independent radiologists evaluated images for lesion number, characterization, enhancement, and subjective preference. Results: There was no statistically significant difference between the two studies for lesion detection or characterization. There was 18% increased lesion enhancement for Gd-BOPTA, compared to Gd-DTPA, of the dominant lesion. Both blinded readers subjectively preferred the images using Gd-BOPTA over Gd-DTPA in the majority of cases, based on greater lesion enhancement and better edge definition. Conclusion: At equivalent full doses, Gd-BOPTA compared similarly with Gd-DTPA in the detection and characterization of focal hepatic lesions in patients with chronic liver disease. However, Gd-BOPTA was superior for increased lesion enhancement and subjective preference of the reader.

Objective: To identify predictive variables of iron overload in patients with sickle cell anemia, correlating biochemical and imaging markers. Materials and Methods: The authors developed a cross-sectional study involving 32 patients with sickle cell anemia who were evaluated for ferritin and iron serum levels and submitted to liver magnetic resonance imaging with one spin-echo and five gradient-echo sequences. The signal intensity was obtained at each sequence, corresponding to the arithmetical mean of the measurements on regions of interest in the liver and paraspinal muscles to obtain the liver/muscle signal intensity ratio (SIR). Based on such SIR, the liver iron concentration (LIC) was estimated by means of the following formula: e^{[5.808 – (0.877 × T2*) – (1.518 × IW)]}, where T2* is the SIR on the sequence with echo time = 13 ms and IW is the SIR on the intermediate-weighted sequence. Patients were grouped according to their blood transfusion regimen (regular monthly versus sporadic transfusions). Results: The comparison between the transfusion groups was based on clinical-laboratory variables, with significant differences in SIR, LIC and serum ferritin levels: the group with regular transfusions demonstrated greater hepatic iron overload. Conclusion: Magnetic resonance imaging is an efficient tool for evaluating liver iron overload in patients with sickle cell anemia.

Objective: To comparatively evaluate the effectiveness of transvaginal ultrasonography (TVUS) and sonohysterography (SHG) in the assessment of endometrial diseases in women with abnormal uterine bleeding. Materials and Methods: Cross-sectional study with 30 patients, aged from 29 to 71 years, 21 (70%) of them premenopausal and 9 (30%) postmenopausal. Saline solution (at 9%) was utilized as contrast agent for SHG. The MacNemar test was utilized for comparison of diagnostic studies. Results: Hysteroscopy diagnosed 18 cases (60%) of intracavitary alterations, and 10 polyps (33.3%). TVUS demonstrated 83.3% sensitivity and specificity, and SHG showed 94.4% sensitivity and 91.6% specificity. The MacNemar test showed similar sensitivity (p = 0.500) and specificity (p = 1.000) between TVUS and SHG in the detection of endometrial diseases. In the diagnosis of polyp, SHG showed the highest sensitivity (90.9% vs. 27.3%; p = 0.016) with similar specificity (89.5% vs. 94.7%; p = 1.000). Conclusion: SHG and TVUS present a good predictive value for endometrial diseases in patients with abnormal uterine bleeding. SHG and TVUS present similar sensitivity and specificity in the detection of such diseases, but SHG is more sensitive in the detection of polyps.

Objective: To evaluate the possible repercussions of the previous cesarean scar at uterine arteries Doppler velocimetry between the 26th and 32nd gestational weeks. Materials and Methods: Prospective cross-sectional study including 45 women between 11 and 14 weeks of gestation, divided into three groups: 17 pregnant women with uterine scar resulting from a previous elective cesarean section (group A); 14 with uterine scar resulting from a previous cesarean section performed during labor (group B); and 14 pregnant women with a single previous vaginal delivery (group C). Uterine arteries Doppler velocimetry was performed with transabdominal approach. Means, medians and standard deviation (SD) were calculated for each group in the study. The groups comparison regarding pulsatility index was performed with the non-parametric Kruskal-Wallis test. Results: In group A, the pulsatility index ranged from 0.60 to 1.60 (mean: 0.90; SD: 0.29), in group B, from 0.53 to 1.43 (mean: 0.87; SD: 0.24), and in group C, from 0.65 to 1.65 (mean: 1.01; SD: 0.37); with p = 0.6329. Conclusion: No repercussion of previous cesarean scar was observed at uterine arteries Doppler velocimetry performed in the period between the 26th and 32nd gestational weeks.

Objective: To describe the results of foam sclerotherapy in the treatment of chronic venous insufficiency. Materials and Methods: Longitudinal study of cases developed in the period between January of 2007 and November of 2009, with 18 patients with venous insufficiency treated with ultrasound-guided foam sclerotherapy in a private clinic. The Tessari’s method was utilized to produce the foam and the length of the patients’ follow-up varied from 4 to 44 months. Total occlusion of the treated vein and complete ulcer reepithelialization were the primary outcomes, and the secondary ones were vein caliber reduction, aesthetic improvement and patient’s satisfaction. Results: The rate of total or partial occlusion achieved 83.4% in the first review (55.6% total and 27.8% partial) and 66% (44.4% total and 22.2% partial) among the patients who underwent a second review. The rate of varicose veins occlusion achieved 80% and the rate of ulcer healing achieved 70%. Conclusion: Ultrasound-guided sclerotherapy has demonstrated to be a safe and effective procedure for the management of chronic venous insufficiency in this group of patients. The incidence of complications was minimal and most of the patients reported satisfaction with the results of the treatment.

Objective: To evaluate the intermethod agreement in the measurement of compression force as well as variations in the testing of different mammography systems. Materials and Methods: Ten compression force measurements were performed for each proposed method on each mammography apparatus (Mammomat 3000-Siemens, Mammo Diagnostic UC-Philips and Alpha ST-GE), with an evaluation of the differences among the mammographic equipments as far as the application of such measurement methods is concerned, as well as the differences among the applied methods. Results: Significant differences were observed among the results for the different systems and methods, and the values measured with the aid of a bathroom-scale-type dynamometer resulted in compression force underestimation. Each compression system presented different responses for the proposed methods. Conclusion: Differences were observed among compression systems and among proposed methods. Thus the choice of the way the compression test is performed is critical for the acceptance of the mammography equipment, the technical lead being the most competent to define the most appropriate and realistic testing method. It is suggested that the compression force measurement should be performed with an extension dynamometer, rather than with a compression dynamometer, in cases where the mammography compression system functioning is not known.

Objective: To validate the proposal for development of a virtual collaborative environment for training of nuclear medicine personnel. Materials and Methods: Organizational assumptions, constraints and functionalities that should be offered to the professionals in this field were raised early in the development of the environment. The prototype was developed in the Moodle environment, including data storage and interaction functionalities. A pilot interaction study was developed with a sample of specialists in nuclear medicine. Users’ opinions collected by means of semi-structured questionnaire were submitted to quantitative and content analysis. Results: The proposal of a collaborative environment was validated by a community of nuclear medicine professionals and considered as an aid in the training in this field. Suggestions for improvements and new functionalities were made. There is a need to establish a program for education of moderators specifically for this environment, considering the different interaction characteristics as the online and conventional teaching methods are compared. Conclusion: The collaborative environment will allow the exchange of experiences and case discussions among professionals from institutions located in different regions all over the country, enhancing the collaboration among them. Thus, the environment can contribute in the early and continued education of nuclear medicine professionals.

The present study was aimed at analyzing civil liability for medical errors specifically for a radiologist’s errors in the mammographic diagnosis of breast cancer. As examples, Court decisions were analyzed with the aim of showing the understanding of the Brazilian Courts of Law, among them the Supreme Federal Court that is responsible for reviewing, on appeal, judicial decisions affronting the Brazilian Constitution, as well as the Superior Court of Justice, that is competent to judge on appeals against decisions violating Federal Laws. The reviewed material demonstrates the Courts’ view on the subject discussed on the present article, describing the consequences of radiologists’ and radiological centers’ errors in the sphere of the civil law that establishes monetary indemnity as a consequence of the existence of civil liability. That is to say that the duty to indemnify arises from the testifying of the existence of civil liability. Thus, once the civil liability is removed, the duty to indemnify comes to an end. Indemnity should not be confused with civil liability; the latter is the foundation of the former.

Short bowel syndrome is defined as the small bowel functional absorptive surface inability to provide adequate nutrition, leading to intestinal failure and chronic malnutrition. In adult individuals the main etiologies for short bowel syndrome are related to extensive or multiple surgical bowel resections secondary to mesenteric ischemia, Crohn’s disease and actinic enteritis. Besides evaluating the transit time through the large bowel, barium follow through may be utilized in the measurement of bowel remnants length as well as in the follow-up of structural adaptation phenomena of small bowel and colonic loops. In patients with short bowel syndrome, structural small bowel adaptation consists in hyperplasia of villi and mucosal folds, which become more numerous, deeper and larger in diameter, as well as remnant segment dilation. Such morphological findings are more prominent and best established in the ileal loops, whose remarkable adaptive capacity has been well documented. Therefore, the knowledge of imaging findings regarding morphological and adaptive characteristics of the small bowel is extremely relevant in the multidisciplinary approach to short bowel syndrome.

The most frequently reported causes of snapping knee syndrome are related to abnormalities of the meniscus, biceps femoris tendon, gracilis and semitendinosus tendons. The present report describes a case of snapping in the posterior-medial corner of the knee related to the myotendinous junction of the sartorius muscle in a 58-year-old male patient with knee hyperextension.

Mounier-Kuhn syndrome is a rare disease clinically characterized by recurrent respiratory infections. The present report describes a case of this disease with analysis of chest radiography and high resolution computed tomography showing increased caliber of the trachea, main bronchi and central bronchiectasis. Such changes, in association with clinical data, suggest the diagnosis.
